For your credit cards put down the date which you took the cards out this will show the examiner that they were long term debts and not incurred shortly before your Bankruptcy.
Any debts incurred within the last 2 years try to figure out the month which you took the loan or card out, for debts over 2 years then the year in which the card or loan was taken out will suffice.
