She will need full time child care for 12 weeks of the year during school holidays and the other 40 weeks I will need to pay for her to go to morning and after school club? How do I break this down on my BR form, shall I work out the annual amount for child care and divide it by 52 weeks?

it is not really what is allowed but rather what it costs.
clearly without the child care you wouldn't work so this won't effect the IPA.
i have had clients who have paid £1200 per month child care because of the situation and the OR has acceoted this after reviewing the alternaties.
they are generally reasonable people provided everything is explained
